Rehabilitation Services

For those facing serious mental health and substance use challenges, therapy and medication alone may not always be enough. Whether you’re recently diagnosed, leaving the hospital, working on relapse prevention, or wanting to regain the stamina to return to work, Community Alliance is here to help you develop the knowledge and skills needed for long-term recovery. 

Our rehabilitation services are not one-size fits-all. We will help you build your own roadmap, identifying where you are in your recovery process, where you want to be and how we can help you get there.

Day Rehabilitation

Gain understanding of your diagnosis and build resilience through classroom and experiential learning.

Our Day Rehabilitation program welcomes participants up to five days a week at Community Alliance’s Center for Mental Health. The program provides a wide variety of classes to support your success in the community, including personal and community living; physical health and wellness; problem-solving; understanding and managing your mental health; coping with daily stressors; building healthy friendships and leisure activities; and personal advocacy and empowerment. Classes are facilitated by our trained staff as well as outside speakers and peers. 

Our dedicated Day Rehabilitation area has a separate, direct entrance with classrooms, a cafe, a full health and wellness area and leisure activity space to socialize with peers. 

We will collaborate with you to identify your needs and establish an attendance schedule that works best for you. Together, we’ll build upon your strengths, establish a healthy routine and achieve the level of wellness and recovery you are striving for.

Community Support

Get help when and where you need it.

Community Alliance provides a range of in-home and in-community services to help you grow the confidence and skills needed to live, work and thrive in your recovery. These highly individualized services are flexible in both frequency and duration.

We will support you in achieving a healthier life by encouraging you to take care of your medical and mental health needs; developing and utilizing a personal recovery plan; maintaining a regular eating, sleeping and medication schedule; and choosing healthy leisure activities. We provide these one-on-one services in your home and other convenient community locations on a regular schedule aligned with your needs.

Assertive Community Treatment

Access intensive, individualized support to build skills and prevent hospitalizations. 

Assertive Community Treatment (ACT) offers intensive mental health support for individuals who need help managing their symptoms outside of a hospital setting. It’s designed for those who may struggle to meet basic needs and maintain personal safety in the community without frequent support and intervention.

ACT’s unique, ground-level approach means our team meets you wherever you are — at home, work, school and in the community. We’re available 24/7 to support you in achieving stability and success, helping prevent further hospitalizations. 

Our ACT team follows an evidence-based national model to deliver personalized treatment and support with a focus on real-life skills. We review your needs and goals daily to ensure the program is responsive to your progress. While intensive, the frequency of our services and visits is flexible to fit your individual recovery plan. 

Residential Rehabilitation

Take your next step toward independence.

If you need additional time and support transitioning from the hospital or another acute care environment, Community Alliance offers a supervised residential setting where you can feel safe, establish a healthy routine to manage your mental health and take positive steps toward long-term recovery.  

Our residential services provide a structured program of daily individual and group activities to help you learn more about your mental health and/or substance use challenges, develop a medication routine, and establish a healthy sleep and meal schedule, all while promoting personal safety, self-care and healthy social and community connections. While in the residential program, you can access other relevant Community Alliance services, including day rehabilitation, employment support, outpatient counseling and psychiatric and medical care.

Our residences, located in neighborhoods throughout the Omaha area, are staffed 24/7, providing both a private bedroom for each resident and a supportive community atmosphere. In the program, you’ll take on roles and responsibilities that support your next steps toward independence and recovery, like paying rent, sharing household tasks and connecting with community resources. 

Community Alliance’s residential rehabilitation facilities are licensed by the Nebraska Department of Health and Human Services as residential mental health and substance use facilities.
