Changes to Our Privacy Policy

The agency reserves the right to make changes to the Notice of Privacy Practices (NPP).  We also reserve the right to make the revised or changed NPP effective for protected health information (PHI) we already have about you and for PHI that is created or received in the future.  A copy of the current NPP will be maintained on this Site in addition to being posted in the agency’s facility. The effective date of the NPP is at the top of the first page.  We will also provide you with a paper copy upon request.  You are responsible for periodically visiting this Site and/or the facility posting to check for any changes.
