Client Rights & Responsibilities

As a participant in one or more Community Alliance services, you have certain rights.  These include:

  1. The right to receive services without regard to race, color, national origin, ethnicity, sex, sexual orientation, gender identity, pregnancy, religion, age, veteran status, genetic information, marital status, familial status, and disability when they otherwise meet organizational and program admissions standards.
  2. The right to receive an orientation to the program(s) in which you are involved including program philosophy, goals/services available, client right/responsibilities, staff responsibilities, program rules and disciplinary actions for violations, complaint/grievance process, and fees as applicable.
  3. The right to a safe environment within Community Alliance’s programs and facilities.
  4. The right to confidentiality of records, communications, personal information and privacy of information related to self, in accordance with applicable law.
  5. The right to be treated with dignity and receive services in an environment free from neglect, harassment, financial and other forms of exploitation, humiliation, retaliation, and any type of abuse including physical, psychological, sexual, and fiduciary.
  6. The right to access your own records and the right to pertinent information in sufficient time to facilitate decision making except as limited by law.
  7. The right to receive services and refuse services without consent or approval of others except as required by law.
  8. The right to informed consent and the right to refuse to sign authorization to release information.
  9. The right to informed consent, refusal or expression of choice, and withdrawal of consent regarding receiving concurrent services.
  10. The right to choose or refuse their providers of service & composition of the service delivery team including expressing preference regarding rehabilitation team members.
  11. The right to expect adherence to research guidelines and ethics if asked to participate in research projects as well as the right to decline to participate in research.
  12. The right to access or receive referral to legal entities for appropriate representation, self-help organizations, and advocacy support groups.
  13. The right to suggest changes and/or file a complaint/grievance; be free from interference, coercion, discrimination, discharge, or reprisal as result of such filing; and the right to a fair hearing in the handling of a complaint or grievance as outlined in the organization’s consumer grievance policy.
  14. The right to investigation and resolution of alleged infringement of rights.
  15. The right to request a written explanation of why they were refused services while in the program.
  16. The right to know if any special interventions or restrictions of rights are used in any program.
  17. The right to be free from physical or chemical restraint.
  18. The right to be informed of changes in relevant agency policies and procedures and charges for care, treatment and services or have their guardian, conservator, or payee receive this information.
  19. The rights as outlined in Community Alliance’s Notice of Privacy Practices.
  20. The right to receive respectful and safe care, treatment and services from competent staff members.
  21. The right to formulate advance directives and have directives followed by Community Alliance.
  22. The right to be informed of these rights at admission and while you are involved in Community Alliance programs.
