Assertive Community Treatment

 

Each individual’s recovery process is unique.  By combining a comprehensive range of community treatment, rehabilitation, and support services into one team, and individualizing those services to the unique strengths and needs of persons served, the ACT program has been found to be very effective in reducing hospitalizations and emergency room visits.

The ACT team is comprised of licensed mental health professionals, nurses, substance abuse and rehabilitation professionals, peer support worker and psychiatrist, all of who share responsibility for the individuals served by the team.  Services are flexible, are provided within the individual’s home and locations throughout the community, and are available on a twenty-four seven day a week basis.




 

 

 






The ACT team meets daily to review the needs and progress of each person being served, and partners with the individual in outlining and helping to achieve long-term recovery within the community. The structure and processes of Community Alliance’s ACT team follows the national model on which it is based.
